Fitness & Wellness Session Terms
You are purchasing one or more sessions of fitness training, lifestyle/weight management consulting, or other wellness-related services. These sessions are subject to expiration in one year. If your Perfect Personal Training, LLC (herein referred to as PPT) professional is to become unavailable for any scheduled appointment, you will be assigned another PPT professional or given an extension on the expiration date. By signing below, you are willing to schedule appointments subject to PPT availability. PPT will reserve your requested appointment time(s) for you and you only. If you become unavailable for any scheduled appointment(s), you may reschedule under the following guidelines:
- You are calling 877-698-DO-IT more than 24 hours before your scheduled time
- Your new appointment is subject to PPT availability
- Your rescheduled date must be within 30 days of your previously-scheduled date
- This is your first time rescheduling this appointment
- This session(s) is non-transferable and non-refundable.
PPT’s professionals are often contractors and are not necessarily employed by PPT. While PPT makes every effort to ensure that our professionals are highly qualified and professionally insured, you may want to request to see the credentials of your assigned professional. PPT does not assume responsibility for any injuries that may occur during or as a result of physical training. Joint injury, abnormal blood pressure, fainting, disorders of the heart or other injuries or complications are unlikely yet potential risks associated with physical activity. Signing this contract acknowledges your understanding of these risks and your decision to begin fitness training, nutrition coaching, or other wellness services. PPT cannot be held responsible for any injuries or for the decisions of your instructor(s), therapist(s), or trainer(s).
PPT protects our assets, trade information, and intellectual property. By signing below, you agree that no non-PPT monies of any kind should, at any point in the present or future, be exchanged with any PPT representative or former PPT respresentative. No outside work should be offered nor accepted between you and any PPT representative or former representative. Evidence of offering or accepting work with a PPT professional will result in prosecution and entitles PPT to full reimbursement of lost revenue and other financial company losses relating to scheduling, advertising, name branding, and client relations. PPT is fully entitled to existing or potential damages, potentially upwards of $100,000 per incident, if non-PPT monies are exchanged or if your misuse of company information leads to loss of PPT revenue.

Commitment Package Purchase Terms

Contract for Weekly Fitness Sessions
Are you ready to commit to fitness with Perfect Personal Training, LLC?
You are agreeing to automated payments. By signing on with automated billing, we reserve your requested time slots for you and you only until you choose to cancel.
You will be charged the agreed amount now. This payment will recur every 28 days. To change or cancel this recurring cycle, call 877-698-DO-IT at least 5 business days before your next scheduled billing. Perfect Personal Training, LLC (herein referred to as PPT) will then cancel or modify your account per your request. Your account billing can not be canceled unless PPT is notified at least 5 business days before your scheduled withdrawal.
Sessions canceled or rescheduled within 24 hours of their appointed time are subject to forfeit. Rescheduling sessions without forfeit is permitted by calling 877-698-DO-IT with a minimum of 24 hours notice.
These sessions are non-transferable and non-refundable.
PPT’s trainers are contractors and are not employed by PPT. While PPT makes every effort to ensure that our trainers are highly qualified and professionally insured, you should request to see the credentials of your trainer. PPT does not assume responsibility for any injuries that may occur during or as a result of physical training. Joint injury, abnormal blood pressure, fainting, disorders of the heart or other injuries or complications are unlikely yet potential risks associated with physical activity. Signing this contract acknowledges your understanding of these risks and your decision to begin fitness training. PPT cannot be held responsible for any injuries or for the decisions of your trainer(s).

Massage Therapy
Congratulations! You are about to purchase one or more massage therapy sessions with Perfect Personal Training, LLC or Perfect Personal Massage (herein collectively referred to as PPT), at your home or other desired location. If you become unavailable for this/these appointment(s) after scheduling, you may reschedule, without forfeit, under the following guidelines:
- You are calling PPT with more than 24 hours before your scheduled time
- Your new appointment is subject to PPT availability
- Your rescheduled date must be within 30 days of your previously-scheduled date
- This is your first time rescheduling this appointment
- This session(s) is non-transferable and non-refundable.
PPT’s massage therapists are often contractors and not employed by PPT. While PPT makes every effort to ensure that our professionals are highly qualified and professionally insured, it is best to request to see the credentials of your massage therapist. PPT does not assume responsibility for any injuries that may occur during, or as a result of, massage therapy. There are unlikely, yet potential, risks associated with treatment. Signing below acknowledges your understanding of these risks, and that PPT cannot be held responsible for injuries or for the decisions of your massage therapist(s).

Contract for Yoga 40 Sessions
You are purchasing 40 sessions of outcall Yoga. By signing this contract and making this purchase, you are stating that you have already scheduled and agreed to your requested times and dates with a PPT professional, or that you are willing to schedule appointments subject to PPT availability. PPT is now reserving your requested appointment time(s) for you and you only. If, for some reason, you become unavailable for any scheduled appointment, you may reschedule under the following guidelines:
- You call 877-698-DO-IT more than 24 hours before your scheduled appointment time.
- Your requested reschedule time and date are subject to PPT availability.
- Your rescheduled date must be within 30 days of your previously-scheduled date.
- This 40-Session fitness package is non-transferable and non-refundable.
While PPT makes every effort to ensure that our instructors are highly qualified and professionally insured, you should request to see the credentials of your instructor at your meeting. PPT does not assume responsibility for any injuries that may occur during or as a result of our Yoga instruction. Joint injury, abnormal blood pressure, fainting, disorders of the heart or other injuries or complications are unlikely yet potential risks associated with physical activity. Signing this contract acknowledges your understanding of these risks. PPT cannot be held responsible for any injuries or for the decisions of your instructor.

- Your new appointment is subject to PPT availability
- Your rescheduled date must be within 30 days of your previously-scheduled date
- This is your first time rescheduling said appointment
- This session(s) is non-transferable and non-refundable.
While PPT makes every effort to ensure that our wellness professinals are highly qualified and professionally insured, you should request to see the credentials of your Yoga instructor. PPT does not assume responsibility for any injuries that may occur during or as a result of our services. Joint injury, abnormal blood pressure, fainting, disorders of the heart or other injuries or complications are unlikely yet potential risks associated with physical activity. Signing below acknowledges your understanding of these risks for those involved in this/these sessions(s) and any future sessions.
